D3P Membership — Plan Terms & Disclosures
- D3P is a discount dental membership plan. It is NOT insurance and is not a substitute for dental insurance.
- The plan cannot be used together with, or in coordination with, any dental insurance. Member discounts do not apply to services that are billed or submitted to a dental insurance plan.
- The plan provides discounts on dental services performed at Viera Dental Design Studio.
- The plan does not make any payments to providers. You are responsible to pay the practice directly, at the discounted member rate, for the services you receive.
- This is not a qualified health plan and does not meet minimum essential coverage under the Affordable Care Act.
1. What D3P is
D3P is an in-house dental savings (membership) plan offered directly by Viera Dental Design Studio to its patients. In exchange for an annual membership fee, members receive included routine exams and X-rays, plus discounted member pricing on other dental treatment performed at our office, without deductibles, annual maximums, or insurance claim forms.
2. What's included at no extra charge
Each membership year, the following are included at 100% — no charge beyond your membership fee — when performed at Viera Dental Design Studio:
- Routine (periodic) dental exams
- Annual dental X-rays
Cleanings are not included free of charge. Cleanings and all other treatment are provided at discounted member pricing, described below.
3. Member pricing on other services
Members pay reduced fees on other dental services performed at our office. Representative member pricing:

Pricing shown is representative and current as of this plan version; fees are set by the practice and may be updated, and the amount due is confirmed at the time of service. Some services, lab fees, or products may be excluded. Member pricing applies only to services provided at our office and only while the membership is active.
4. Membership fee, term & renewal
The membership fee is $125 for one year. Fees are set by the practice and may change; the exact amount you authorize is shown at checkout before you pay. Your membership is active for 12 months from your enrollment date.
This is a one-time charge — your membership does NOT auto-renew and your card is not charged again automatically. As a courtesy, we will email renewal reminders at approximately 60, 30, and 15 days before your membership expires (to the address you provide), so you can choose whether to renew. To renew, simply enroll again for another 12-month term; if you do nothing, the membership and its benefits end on the expiration date.
5. Cancellation & refunds
Because the membership is a one-time annual charge that does not auto-renew, there is nothing recurring to cancel. Refunds are at the discretion of Viera Dental Design Studio. Any unpaid balances revert to full-fee (non-member) pricing after 90 days.
6. No coordination with insurance or other offers
The D3P membership cannot be combined with dental insurance, workers' compensation, or any other discounts or offers. If you have dental insurance and choose to have a service billed to that insurance, the D3P member price will not be applied to that service. You may not "stack" the plan price on top of an insurance-billed service or another offer.
7. Eligibility, membership & general terms
- The plan is available to patients of Viera Dental Design Studio who do not currently have dental insurance.
- Membership covers a single named person and is not transferable. Family members may enroll at any time; each person requires their own individual membership.
- The plan is exclusive to Viera Dental Design Studio and cannot be used at any other dental provider.
- Payment for treatment is due on the day of your appointment (cash, check, or major credit card).
- Member pricing has no cash value and applies only while the membership is active.
- The practice may update these terms and the member fee schedule; the version in effect at your enrollment or renewal applies to that term.
