Your membership is exclusive to Viera Dental Design Studio and is an annual Plan.
Disclosures
The Viera Dental Design Studio Discount Plan is available only to patients who are not using dental insurance. It does not qualify as insurance, prepayment for future dental work, payment for access to discounted services, or a dental plan under the Affordable Care Act. Our plan allows patients to make yearly or monthly payments for preventive dentistry. The plan takes effect as soon as the patient enrolls and will remain in effect for 12 months, with automatic renewal on the date of enrollment. If you are paying month to month, you are required to pay each payment in order to remain a member. You may cancel your renewal at any time but will be required to pay any outstanding membership fees and unpaid balances. Any unpaid balances will revert to the full fee after 90 days. Refunds are at the discretion of Viera Dental Design Studio.
This plan cannot be combined with insurance, worker’s compensation, or any other discounts or offers. If you elect to have additional services, payment is due on the day of your appointment. The plan cannot be transferred to any other provider.
